More than half of the patients with cardiovascular diseases in Russia are treated incorrectly
In our country, the blood supply to the heart by surgery often do not recover to those who need it, but do the operation for those who don’t need them.
Researchers from Saratov state medical University and Saratov branch of Institute of Radioengineering and electronics of RAS was evaluated in Russia are the European guidelines for revascularization (medical intervention aimed at the elimination of the deficit of blood supply to the myocardium) in patients with coronary heart disease (CHD). They found that in a significant number of cases, surgery does not perform to those who according to international standards are shown. Scientists took data from the Russian register of patients with hypertension, ischemic heart disease and congestive heart failure. They included 1522 patients that in 2012-2015 was listed in the register and had a diagnosis of “stable ischemic heart disease”. The average age of the patients was 53 years and the majority (76.2%) of them were men. 591 person (38.8%) held the surgical restoration of blood supply to the heart surgery for revascularization, such as coronary artery bypass grafting, as did the first President of Russia Boris Yeltsin. And 931 (61.2 per cent) refused revascularization.
The correctness of the decision that the patient needs surgery, checked on the six criteria listed in the guideline of the European society of cardiology and the European Association for cardio-thoracic surgery. Under these criteria fell 1196 people — 78.6% from the whole sample. However, it turned out that only 481 the man actually performed the surgery for revascularization. They were among 591, mentioned above. Thus, surgical intervention was subjected to 110 people, which it is according to European parameters was not necessary. And of those 931 patient who did not consent to the surgery it was needed most — 715.
According to this statistics, more than half of the cases of people with coronary heart disease treated incorrectly: do not do operations to those who for objective indicators of need them and Vice versa. However, the received data has a problem. Russian register of patients with arterial hypertension, ischemic heart disease and congestive heart failure makes it impossible to see the contraindications patients. Probably many of those who needed coronary artery bypass grafting, it is impossible to do for some objective reasons. So blame Russian doctors for incompetence on the basis of the survey data is not necessary, despite the fact that they clearly deserve attention.
